Random picture message has no meaning, or does it?

This came in on last Wednesday. There wasn't a note, no description. I wrote the sender back asking for details. Nada.

At first, I'm thinking, ok, bag rider (which is a topic that has been beaten to death) but then, wait ... what's that behind the woman's head. A bag? On the headrest?

Is it?
