If you look very close in the second picture, the light standard on the far side of the intersection has a clearance restriction in the left lane due to low tree branches. There is no such clearance restriction in the right lane, so it looks like driver error.
That almost happened to a GO Transit DD last September when a report driver violated the geo-fence, but the alarms didn't sound. Fortunately, a regular driver, who was monitoring the radio, recognized the imminent disaster and commanded the other driver to stop. $800,000+ saved.
